Intergrated care pathways

Dear all
My colleagues and i are trying to develop intergrated care pathways for the following procedures
Carotid endarterectomy
AAA
EVAR
lowerlimb bypass
amputation
VVS
I wonder if anybody out there has any examples of these pathways that they will be willing to share.

Hi,
We have no pathways as yet, but in York we are just starting on one for amputees. I would also be grateful for any suggestions/ drafts etc
